One of my friends was in the same situation, her husband was the primary while she was the derivative(dependant). She was pregnant and hence could not take the shots, I think their lawyer made appropriate notes before filing I485,
However, here is something that you can do, in some cases ( not all) the vaccines are not even required,
for e.g me and wife got the blood tests done to determine what vaccines are needed to be taken, this turns out to be a little expensive if you are paying out of pocket. But it avoids the unnecessary vaccines especially when your wife is pregnant/breastfeeding. This options should be considered especially when you are planning a baby/pregnant/breastfeeding, the INS doctor may OR may not speak to you about the tests before vaccinations.
In my situation, my wife came negative with everything and hence she did not have to take "any" of the vaccines, the INS doctor included the test report stating that she did not need vaccines, as for me, i was required to take just "one" vaccine instead of the regular 5-6 vaccines.
The total cost for everything for both of us ( tests + vaccines + chest xray (for me since i failed the TB skin test) ) came to around 1000+, however, this totally depends upon the doctor you go to.
I knew that the INS doctor that i went to was taking me for a ride, but i did not care since my employer was paying the expenses.
