Does anyone have sample for Affidavit for one and the same name?

Hi all,

Does anyone know if there is any sample for Affidavit for one and the same name available online?

I need it to create for my father's name, which is written as Firstname and then "Bhai", "Kumar" at the end of the first name and I want to create affidavit by my sister or other relatives to declare that all these names are for one and the same person.

Your question is not clear. Can you explain in better English?
What do you mean by "for one and the same name"?!?
