Does anyone have a copy of a Non Availability Certificate from the Consulate of India San Fransisco

I just helped my mother move and a bunch of papers have been mixed up. I need to know how a Non Availability Certificate from the Consulate of India San Fransisco looks like. I have been searching through papers all morning and cannot find anything and knowing how it looks would help very much.

If anyone is kind enough to scan it in, I would be very grateful.

This is in order to get my brother to the States, we need proof to send that my mother does not have a birth certificate, and I need to find this asap.

That certificate is useless for US immigration application and will not be accepted by USCIS. You need a letter of non availability from the official registrar (village panchayat or city corporation) of births and deaths form the place of birth and also 2 affidavits form close relatives attesting to being aware of your mother's birth.
