Documents to Attach for AP Filling

Hi
I am planning to apply for AP for me and my wife with out going through Lawyer.

My question is: -

1) what are the documents that I need to attach when filling AP?
2) Could some one post a sample format letter for the reason for AP (visiting parents, family function etc..) or point me to an URL which has a sample format?
3) Do I have to attach a copy of my previous AP (expires 5/3/03 - never used it)

485 copy
previous AP copy
Copy of passport 2 pages + 194 copy ( does't require but no harm in sending)
140 approval copy
Form 131 + 2 photograps immigration style and alin number on there back

For wife above all + marriage certificate.
