Documents required for EAD and AP Renewal

Please let me know the documents required for EAD and AP renewal. I checked USCIS website and also prepared I-765, I-131 and all required additional documents. Just I would like to confirm before mailing to USCIS.

BTW, There is some search problem in this forum, so please don't ask me perform a search.

This is what I remember sending

1.)Front and back of I-94 card
2.)Front and back copy of your last EAD (assuming you had one before).
3.)Copies of your previous AP if you had one before .
4.)ADIT style pics. as applicable.
 
Also, for EAD, if have the original approval, tear off the top portion and send it with renewal application.
 
and attach a copy of ur pending I485 Application..!
and Forget NOT the Fees
and a Letter for applying AP..!
 
I sent copy of passport, drivers license for my spouse renewal. I got a RFE for my case for the above.
 
One more question.

Do I have to write check in favor of INS or BCIS or USCIS or DHS ?????? I remember reading somewhere all those names are acceptable but I still prefer to confirm the same.
 
I-140 approval notice

You had better your I-140 approval notice.
Many people at the CSC forum had got a RFE for I-140 approval notice even for EAD renewal.
I attached it to my dependent's EAD renewal, and I had no problem.

They are still accepting the checks in favor of 'Immigration And Naturalization Service'. You can write check on this name.
 
I-140 approval notice (correction)

A copy of I-140 approval notice is enough.
