documents needed for 140/485 filing

Guys,
Can someone suggest me what all documents needed for filing 140/485

I will get my approval notice in 1-2 days and I want to make sure sure that I will collect all the docs by the time I am ready to file.
Since, we can file simultaniously, it will really help me if someone let me know the list od docs, either seperately or combined, 140, 485...

For I-140 - nothing really since its a petition by the employer for the employee so your company will provide all the docs.

For I-485

Docs and Information needed :

1. birth certificates ( self and family)
2 marriage certificate ( if applicable)
3 vaccination records if available.
4 Green card style photos.
5 Medicals need to be done from an INS certified doctor ( #3 is helpful in this )
6. Birth dates/places of parents.
7. Residence and Employment history of the past 5 years.

these are what i could think of at this time.

Any idea for what all vacinations we need record of?
Also, do they ask for proof for parents DOBS and places? or its just that we write out of our knowledge?
 
vaccinations :
in my case they asked for MMR, Td, Varicella.
Get records of other vaccinnes too if applicable..

DOB:
I do not think they ask for the proof.

( I mentioned this item because generally not everyone remembers it and to get it from your folks in other country might waste a day or two..so its better to have it in time while preparing)
 
Document needed to Withdraw I140 application

Hi
I am new to this board and just wanted to know what is the procedure to withdraw the I140 application from INS(California). My company is planning to withdraw one of my collegues application(since he left the job) and want to subsitute his labor with mine. So I wanted to know whether there is a form to be filled up and sent to INS to withdraw the I140 application? or the procedure to withdraw the application

One more question?

1)Do the fathers name on the passport has to be exactly same on the birth certificate too? suppose passport has fathers name as like Kovi.v.Rao
can it be Kovi Venkateswara Rao on the birth certificate?
2)Suppose if the Birth cert is missing and if we need affadavits from parents, do they have to be on stamp paper or regular paper?
