Documents needed for 140/485 Filing?

Hello,

I would like to know what documents (checklist) are needed for 140 and 485 filing and which documents are needed to be notarized.

I don’t have birth certificate. My Date of birth is mentioned on 10th mark sheet and passport. Does that work? Or how can I arrange birth certificate for filing 140/485?

W2s-copy of all W2 Employer Wage Statements for the latest tax year
Pay stub-copy of your most recent pay stub

485

For Consultant:

Signed forms (I-485, G-325A, I-765, I-131, and G-28)
Completed Medical Exam in sealed envelope
Photo Identification--Two (2) clear copies of your driver’s license or other photo identification (biographical page of passport, etc.)
Copy of IRS 1040 Federal Tax Return for the latest tax year
Copy of all W2s Employer Wage Statement for the latest tax year
Copy of Birth Certificate and two affidavits of support (if necessary)
Copy of Marriage Certificate (if applicable)
Passport-a legible copy of every page of your passport, including all visa and immigration stamps.
Form I-94-a legible copy of the front and back of the I-94 issued to you at the time of your most recent entry into the United States

Form I-797 Approval Notices-a Copy of all current and previous Forms I-797 approval notices for any petition submitted on your behalf

Six (6) Photographs

Other-If previously a student in the US, copies of the front and back of Form I-20, Form IAP-66, and/or EAD card

For Spouse:

No, you CANNOT use the 10th marks sheet, you would need two affidavits and a non avail certificate from the muncipal corporation where you were born

Someone told me that the affidavits and non-availability certificate must be issued within six months when you file I485 for all the members of family. Is this true?
 
not heard anything like that, but at times it is also possible to register the birth late at the municipal office and have a BC issued. I jknow ofcases like that.

Never heard about any such rule, check with your lawyer
 
The six- month rule seems like a rumor to me, if the municipality is sissuing a non-avaiability certificate today that they cannot find the birth registered then how can they few mths or year down the road.

Same with the affidavit, as long as it is sworn and notarized it should be fine.

U do not expect the BC to expire every six mths do u.
