Documents for Baby Canadian PR ??

I appreciate if someone can advise which Application form and what other documents do I need to send to the Canadian Immigration to file an application for my recently born child as a dependent to my application?

He was born after we had completed My wife's and my medical exams.

Though I have a feeling it could be his birth Certificate, his American passport, FBI clearance (for a 2-week baby?? Not sure), $$ application fee (Not sure how much??), Some forms (Form Number not sure??), Medical Forms, Immigration pics (# not sure??) .....etal.

Hi!

We had our baby before the medical exam but we filed the following papers:

Application (sent to us by Canadian Immigration in response to our letter about Change in Family)

Application fee ($65)

Copy of the passport

Copy of the birth-certificate

No FBI clearance was needed

However she did have a basic medical exam when we went in for ours.
