Documents for 3rd EAD renewal app

Hi all,

I have a RD of Apr 02 and I am planning to apply for my 3rd EAD by end of January as it's taking about three months for EAD approval.

I wanted to ask a few questions regarding for I-765 and attachments:

In Question 11, what should I fill for "Dates(s)" field? Both dates for first and second EAD applications or just the second Application date?

Also should I attach documentation (copy of EAD/approval notice stub) for both first and second EADs or just the latest EAD.

Also those who have filed for thier third EAD renewal, please let me know if there is anything different that you did from your second EAD renewal application.

You should've asked these questions in the main forum.

You can specify the dates of your last EAD and attach the copies of front/back of your last (2nd) EAD. Make sure you attach the top portion of the last EAD approval notice.
