documentation of salary with I-140 for EB1-EA?

Hi,
do I need to provide any documentation regarding my salary when filing my I-140 under the catagory Eb1-EA (self-petitioned)?
E.g. something like a W-2 form etc?

I am not certain what to indicate under wages/week in the proposed employment if one is applying for Eb1-EA that is not employer-sponsored?
Should I simply put in my current salary?

Also, how relevant is the question if the position is permanent?
