Document translations and affidavit

Hi All,

Had a couple of questions.
1. I would need to get my Birth cert. and Marriage cert. translated.
Apart from the translation, what statement must be added by the translator, and should this statement be on a separate sheet and attached to the translation? Or can it be on the same sheet?

2. Since the BC does not show mother's maiden name, is it necessary to get an affidavit from mother stating her maiden name (which is on DS230 pt 1?). If so, does anybody have a format for this?

Here is format thaat I used to get cert from translator.

Certification by Translator

I, _____________, certify that, I am fluent(conversant) in English and _______ languages, and that the attached/(above) document is an accurate translation of the document attached entitled Certificate of Birth/MoM.

Signature: Typed Name and Address:

Date:

REMEMBER, you have to get it noterised by translator. It may be on same or separate sheet. If on separate sheet then initials should be on translated doc.

You need affidavite for missing mothers name on BC.
here is format that I used -
AFFIDAVIT BY name of mother/father

Re: your name

We/I mother/father, hereby declare that :
1. Our/My full and complete address is
2. That I, father was born on ____ in the _____, India.
3. That I, mother , was born on ____ in the _____, India.
4. We/I married on _____ in the town of ____, India.
5. We/I father/mother had the following children:
a. your name (Child's Maiden Name: ____) born on your date of birth; and
b. name of other child/ren
6. Your name was born in ______, India.
7. This affidavit is being submitted because there is no official record of mother’s name on birth certificate.
We declare under penalty of perjury, that the foregoing is a true and correct statement.
