Folks - Do we need to include an additional page of the receipt notice when sending faxes?

I don't think its' required as long you use the standard fax sheet. In the fax sheet you will be able to mention your WAC number.

nice if you do, it will help them track down the case in case of any mistakes in entering WAC number etc. on Fax form. Also attach a letter explaining the problem. There is no place on the fax page for a detailed explanation which might be essential in your case.

I think it is good to include a letter, I-145 receipt and I-140 approval along with FAX sheet. I sent fax couple weeks back, i got 2nd FP Notice 06/01/2004.
