Do need still need to submit I612 even after DOS sent the favorable recommendation to USCIS?

I am confused, each time I called the USCIS CSR I get different information. Today They told me probably I need to submit I612 with USCIS

my J1 wavier is based on No ojection Statement from my embassay in WA.
I did not receive any information that I need to file for I612 with USCIS

Do I need to submit I612 with USCIS? So that they can process my case? I have not receive the receipt nos and USCIS CSRs are not able to locate the number in their system.


DOS sent Recommendation to Vernmont on 23 July 2012



Can anyone please advise me base on your knowledge?
