Do I need I-134 form for Eb1b?

Hi everyone, I am new here. I was wondering if I need I-134 form for Eb1b application. I can't get a definite answer from the other forums. Some people say yes some people say no. I hope someone can help me here.

Also, if my wife is the co-applicant in the Eb1b application, does she need a I-134 form? Thank you guys.
 
I think I will go with 485-route. Is there any difference between the consular process and I-485 process in terms of forms? Thank you.
