Do I need a lawyer for O1 visa at NIH?

I've been on J1 for my residency and fellowship and planning to go to NIH for bench research for a few years.

They do sponsor O1 for clinicians who have been on clinical J1.
They have an International Service Division and some 'immigration specialists' who will submit petition for O1.

Do I really need a lawyer, then?
What would they do? Organizing the paperwork and making them look better?
Are they worth the money?
