Date on medical form

Hi everyone i received my RFI requiring medical but the letter says must use form I 693 revision november 2005
this is confusing am i supposed to wait till nov 2005 the new form on the net is sept 2005 revisions and revisions are usually done after years
I called all the doc offices in the area they are all using the sept form I checked with the immigration 800 no as well they say there is no nov revision

need help should i wait for nov or just do the medical on the sept form

I received RFI a couple days ago. My letter says the same thing as your. I am confused with the form november 2005 date.

Ok since you already called the 1800 number its fine. Go ahead and do the medicals. If you go to USCIS's website, you can see that it is written that they still accept FORM I-693(9/16 or prevoius forms acceptable). I think it should be ok if you use the forms.

thx wantaGCnow. I did call again & talk to NSC.The lady says thats an error on the letter. Consider it as Sep 16, 2005 revision form(I693).
Also can u tell me how to fill out this G325A as it is impossible to type on this form on the computer.

I did mine with no trouble what so ever. Don't know where you find the problem? But if that's the case & u can't solve it my suggestion would be just print a copy & fill out using a pen.
