Date of Birth Certificate

Hi All ,
I have my DOB (long form ) issued by government but it only had my father's name on it not my mother name so will this certifcate works or should i have to take Affadavit of Birth certifcate again .

Please clarify .

It is required that BC should have names of both parents.If its possible to get another certificate with both names on it, its fine otherwise take two affidavits one from each parent or close relative stating your birth day( in the prescibed format which u'll find in the forum) and that with your current BC should be enough.

Is your name on the form? If it is, then it should be OK, but take the following additional docs:
Affidavits of both parents, on Rs 20 legal paper. This is easy to get.
High school certificate, showing your name, and birthdate.

The above 2, combined, should suffice.

But if your name is NOT on the BC, then you need to get it again.
 
Could you please send me the sample text in Affadavit of Birth Certificate should have ?
