The application says to use mm/dd/yyyy format but the instruction book says to use mm/yyyy format.
Which is correct?

thanks in advance.

If you have the exact dates, use the mm/dd/yyyy format. I don't think it really is going to be a big deal if you use one format over the other.

Thanks JPBoston. You are great.
another question: Part 9: Information About Your Children:
B. Instruction says that in last column "LOCATION" , write "with me" if they stay with you or write the address. The form does not have a column LOCATION but address.
My kids stay with me. My daughter is in dorm (in same town), should I write WITH ME for both of them or write the my address again.
I know it is simple question, but wanted to be double sure.

thanks

For your child who is staying with you at home, write "WITH ME" (this is what I did for my own child on my app) but for your child who is residing in a dorm, I would put that dorm address since she is in college and not living with you even though in the same town.

Thanks JPBoston.
SHe will be home this month end and leave dorm so that address will not be valid. As I am applying for her N400 also, I have to give my home address as during summer she is at home. So in that case, what should I do "WITH ME" or the dorm address. And my wife also have to write WITH ME too?

thanks

As I am applying for her N400 also, I have to give my home address as during summer she is at home.
Seems like her primary address is your home. So your home should be listed as her address on both your application and hers. The dorm is just a temporary accommodation for the purpose of studying.

College students who stay in their parents house in the breaks between semesters normally are treated as having their primary residence at their parents house.

Note that you should not be applying for her N-400. You can assist, but the N-400 is her own application and she will bear responsibility for any mistakes or misinformation on it.
