CSC fax inquiry form needed

Hello,

I called up IIO this morning to check my spouse's EAD case which was ND 04/17/02. He suggested me to send fax inquiry. I have ever seen the fax inquiry form at www.elirich.com/csc_inquiries.pdf, but looks like it is not there any more. Does anybody know where I can find it?
BTW, my case WAC-01-294-5XXXX has not been assigned yet from IIO. And he kept saying that they are handling the ND 09/01/01 cases.

Here is inquiry form that I had downloaded sometime back. - download the attachment. (it is a .pdf file. I had to rename .pdf to .doc to be able to upload it. Please rename it to csc-inquiries.pdf to be able to view it).

I am not sure if this is still current format of the form. Someone who had recently faxed, may help in this.

Here it is. Rename it to csc-inquiries.pdf from csc-inquiries.doc.
