I am a DV-2007 winner. Have been on a dependent visa to USA previosuly. I want to know how to answer the following on DS-230 as I have to fill the forms for myself, my husband and kid
I couldnt find a guide to fill in the forms and hence asking the questions here.
1. Item 2 - Should I mention only my maiden name OR give maiden name and other aliases used as the form says 'If married woman, give maiden name)
2. Item 12 - Your present occupation - I just stay at home so should I mention 'None' or 'House-wife' ?
3. Items 15-18 - Details of Parents date of birth (date of birth, place of birth, etc...) - Can we put approx details if we dont know while sending the form. Will any proof be asked at the CP interview to prove the details (DoB, place of birth, etc...) of parents ? Or should we be very accurate because when we become citizens and later apply for GC for parents - these details will be referred to & comes into picture ?
4. Item 20 - Do we list all cities/ towns where we have stayed for more than 6 months CONTINOUSLY or 6 months in TOTAL AT ANY TIME. For eg. (3 months in one year, 4 months in another totals to 7 months which is greater than 6) ? Do we have to get police certificates for all cities we stayed in or only those stayed in CONTINOUSLY ?
5. Item 29 - For my form I understand that I put my name/ address as the petitioner. What about on my husband's form and kid's form ? On my husband's and kid's form do I put myself as petitioner OR I put their names only.
6. Item 33a. My husband has a SSN. So - for my husband's DS-230 form - Do I fill in a 'Yes' to question 'Do you want SSA to issue you a new card ?' OR is it ok if I say 'No'.
7. Item 33b. If I answered No to 33a 'Do you want...' - should I answer 'No' here. Will this affect my application in any way ?
8. Item 34. For my kid on DS-230 - I guess I need to put a 'Yes' to the question 'Were you assisted in completing the application' ? So - do I have to sign on the first page of DS-230 OR put finger-print of the kid?
