Dear Member,
Please advice that do I need the PCC from Indian Consulate and Local Police Station in India as well? Also please provide me the sample Birth Certificate becasue there is no goverment record for my birth. Thanks
 
PCC: If you get one from a Indian Consulate in USA then you dont need a PCC from local police station in india. You need one or the other not both!

Birth Affidavit: Try to get a birth certificate or if that is not possible then get a non-availability certificate of birth along with birth affidavits from two family members/ friends/ relatives which need to be on stamp paper (Rs 20) and notarized by a lawyer.

Alter the attached birth affidavit template appropriately to meet your needs!

Divekar, Thank for the advice. Is there any requirement in CP that Birth Certificate should be issued by any specific authority (designation) in India.

BC's / Non-Availability Certififcates are generally issued by Municipal offices in India. Like for instance Mumbai/ Bombay Municipal Corporation (BMC) or such equivalent organizations in different cities in India. So contact Municpal office of the city you were born in and they should be able to guide you in the right direction!

I was reading through one of the thread replies and i came across a very unfamilier term called "Name Check Procedure"
I have never heard of any such procedure.
Would you shed some light on this?

Name check is a procedure where (my understanding is ) the NVC & / or consulate runs a check for primary and derivative applicants name against names of people who have established a criminal background. Normally takes under 5 minutes but if a match is found which happens in a few instances then it takes some time to resolve the issue as one may have to prove that he/ she is not the individual of concern.

Also other scenarios of name check is where all of a sudden a person changes name in the passport for reasons other than marriage.
