I came to know that one should have proof of vacination of following vaccines.
I was born in India many many years ago and I do not have my childhood shot record.
Now what can I do? Will it cause problem in CP medical exam?

Along with the physical exam, immunizations(vaccinations) in accordance with the Center for Disease Control are also required. Panel physicians who conduct medical examinations of immigrant visa applicants are now required to verify that immigrant visa applicants have met the new vaccination requirement, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccinations:

measles, mumps, rubella (MMR)
polio
tetanus and diphtheria toxoids(DPT)
pertussis
influenza type b (Hib)
influenza
hepatitis B
varicella
pneumococcal

No need to worry about it. If you do not have it you might have to retake it.

The vaccinations will differ based on the hospital/city you are going.

alren,
Do we take it at the time of the medicals or prior to that? And are the medicals to be taken only after receiving at letter from the consulate?

The sequence is, first lab tests and then the visit to the physician.

The physician determines the vaccinations you need based on the lab tests.

The lab and the doctor's office would expect to see the interview appointment letter. So, these need to happen after you receive your appointment letter which is part of 'Packet 4'.

What is your consulate? Mumbai ? Then it might be a bit different from what Raju said.

Try discussing in the "sticky" thread for Canada at the top. You will get a more clear idea.
