Could anyone give a list of documents for applying for AP?

Look at page 4 of the 131 form for general filing instructions. What you need to send depends on your status.
 
I E-filed self this year for me and my wife and the documents i sent are
1) Confirmation receipt copy after successful E-FIle as a cover page.
2) Copy of I-485.
3) 2 Passport size photos taken at Costco.
4) I-140 Approval copy.
5) Driver's licence copy.

I got approval without any problems.
