Copy of I-693 (medical forms)

Quick question- do we get a copy of the I-693 forms from the doctor? On USCIS's website, it mentions 2 copies going to USCIS, 1 copy for the doctor and 1 for the applicant. Is that the case? I have asked my doc but he refused. I can get a copy of my medical exams though but not the official I-693.

Actually I was given a copy of the I-693 by the doctor which was for me and one sealed envelope each for me and my wife. Is there something I am missing here? I did not request the copies they just gave them to me when i paid for my visit. I thought this was normal.....!!!

I work in the Medical Services here and I feel no doctor can refuse you documents regarding ANY medical tests that have been done on you. Its right to Privacy act I think. You can officialy request a copy of the I-693 and I think the Civil Surgeon's office HAS to issue one to you.....I inquired about this.
 
I second kulbikr. I received a copy of my I-693 (together with supplemental form) as well. You should be able to get copies of all of your medical records!
 
I asked for copies for me and family, but doctor refused. No copies from sealed envelopes, and no copies on what was done, nothing at all, just receipf for payment...
Later we had problems, turned out that doctor forgot to mark a box in forms inside envelope.. we had to go back and ask for copies, it took them a month to find archived copies and then they again sealed them, I asked again for copies and this time I got them.
So probably it depends on the doctor...
 
show the doctor the instruction at the first page of medical form. It states one copy must be given to the applicant. Report to USCIS the case.
 
Our doctor's office has refused to give us a copy of the completed I-693. I mentioned the USCIS instructions but to no avail. The only thing I am afraid of is the doctor missing some information and then getting an RFE about it. That will be a waste of time.
 
You can go to your local consumer redressal forum. Or can even contact a lawyer. You have a legal right to that copy of your medical form. Ethical and legal. There is no medical test done on your body that can be with held from you. This of course if you are really interested in getting it ...:-)). You can tell them you need the test to show to your work place because they need these results. Or conversely if you have a primary physician their office can officially ask for your tests via their offices. AGAIN YOU HAVE EVERY LEGAL RIGHT TO THAT TEST PAPER.....YOU CAN TELL THIS WITH CONFIDENCE to whoever refuses you this. Politely ask them how would they like to give it to you....via an official request in writing or a legal notice from an attorney....!
