confusion on affidavit of support forms I-864/I134 ?

Hello,

I am preparing to file I-485. [NIW-140 is still pending]

My wife is H4. For her I-485, I need fill out the affidavit of support.
There are two kind of affidavit of support forms.

I-864 form
I-134 form

I see someone [greencardapply.com] say I-864 form and someone [MIT international office] say I-134 form.

Anyone know the exact answer?

Employment-based uses I-134. That's what I used for my wife and son.
