Company name change - Filing Pro-active I-140.

Hi
  myI-485 is on hold, because of company name change. I am tired of waiting for INS guidelines and decided to file Pro-active I-140. Since my company don\'t pay for this, i want to file myself. Can somebody tell me what I need to send with I-140 application? Do i need to send old I-140 approval notice? Do i need to put cover letter explaining why i am filing I-140 again? Do i need to include my experience letters agian? Can somebody answer these question?

I think it would be safer for you to go through your company\'s attorney, because he has all the paper work already. If the employer is unwilling to pay then you can pay to the attorney by yourself.
