"Class of Admission" on AP application

Guys,
What should I enter for "Class of Admission" on the AP application? Are they asking for the category in which I last entered the US? Like H1 or Parolee etc.
Also, do I need to attach any document other than copy of 485 receipt, photographs, letter & the fee?

Yes. I did put 'Pending Status Adjustment (Parole)-485'. It did get approved last time. No other documents were provided other than what they have in the instructions.
