I received my bachelor degree of medicine, master degree of medicine and doctor degree of medicine from a Chinese medical university. Now I am planning to apply GC through NIW and OR. Should my degree be translated to M.D. & Ph.D or just M.D? Also can I just use the notarial certificate from China (with English translation), or should I do the evaluation? I appreciate your kind suggestion. Thanks.

It should be equated to MD, PhD, IMHO. Try Global at http://www.gcevaluators.com/. It will be helpful to have evaluation done by them or any other accerdited agency.
