changing from Visitors Visa to H1B Visa

My brother, a doctor, has visitors visa and it is valid for 10yrs. He recently visited USA, and gone back. Now a hospital is ready to sponsor him for H1B visa.
My question is -
* is it better for him to come in to USA on Visitor's visa, and start the H1B process, and then change to H1B.
* is it better for him to complete the H1B process, and then enter in to USA on H1B.
Out off two, which one is safest.
As I said he is doctor, and has visitor's visa.
 
He should do it the correct - and legal - way. The sponsor should apply for the H-1b, and he should get proper stamping and enter on H-1B.

Attempting to enter on B2 might result in a NO EOS/COS which would require his return and stamping.

Why do people insist on trying to take shortcuts around US immigration law? If he in unwilling to comply and do things correctly, he needs to not come. He intends to work - not visit. I suspect the hospital would be less likely to hire someone - especially a doctor - whose ethical standards are suspect.
