Change of Address (Immigration Court)

Hi Anyone can help me please

I don't understand, how to fill up Alien's Adrress Change fForm/Immigration Court.
Becase there are two place for sign up. SEcond one for whom?
It says "Proof of Service" so, they will prove it and send back to me?

It was esay to complete USCIS's Address change form.
Sorry I am just learning

I am personally not familiar with the Change of Address for Immigration Court. Can you possibly scan the form or let us know what is on the form so that we can assist?
