Change of address form question [AR-11]

Hi ,

On AR-11 form for change of address, the last columns are :

PORT OF ENTRY INTO U.S. & DATE OF ENTRY INTO U.S .

I am ‘almost’ sure that I should enter the latest port of entry and date and not the first (meaning the first time I ever came to U.S.) , Can someone please confirm that I am on the right track?
