Change of Address after GC

Hi

I was granted Perm residentce status on Oct 2005 (marriage to US Citizen). My GC expires in Oct 2007. I recently moved to a new apartment and I would like to notify change of address to USCIS. What form should I use?

Thank u sir. I did review the AR11, and I had one question - about port of entry. what should be the answer - is the port of entry that i first came in to the US when i was on H1 visa (in 2000) or is the port of entry when i returned from india after vacation in Dec 05.
