Certification of good conduct - fingerprints rejected twice

I have applied for my mother's GC while she is currently here in US on visitor's visa. Her first finger print got rejected as the prints were not good (due to old age). Gave another finger print and got a letter that it is rejected as well as "illegible" (basically due to old age again the prints are not good).

Now we need to provide a letter issued bt rge local police dept indicating that she has "no criminal record". A separate letter is required for each law enforcement district in which she has stayed in last 5 years.

During last 5 years, she has lived in India, 2 different cities in USA as a visitor and one city in Japan as a visitor.

Note that the stay as a visitor in US and Japanese cities were less than 6 months. Do we need to get the clearance from all cities in 3 countries? Or is it enough to get a clearance from current US city where she is residing? Do we need to get certificates from India?
