cards reveiced, name incorrect

Hi,
we had our passports stamped at dallas office on March 8th
and we received our cards this weekend. My spouses last name
was incorrectly spelled in the card. Now it seems we need to
file form I-90 to correct this. I have following questions. I appreciate
your help if some one with similar situation share their experience.

1) Can we mail form I-90 or should we need to go in person to
the local office? (the form instructions says to file inperson at local
office).
2) Do we need to submitt Photos etc. again?
3) what evidence do they ask for the correct name? (passport, drivers licence are enough?)
4) Has anyone tried e-filing in this situation, if so, how good (or bad) is your
experience.

My wife e-filed. Got the biometrics appointment by following the instructions on the e-filing. They only asked for the I-90 application, and to see the old card, and passport. They took the photos on the spot, fingerprint, and signature, so no need to bring photos. They gave the old card back to her(which seems to be an anomoly, because everyone else on this forum had thier cards taken away). The new card arrived in about 1 month.

It should work either way - mailing or visiting district office in person. But I would recommand to to visit in person so that you can you can sort it out on the spot or explain if required. Set a INFOPASS appointment, fill up I-90 beforehand and visit office.

Yes, new photos taken within 30 of appointment date.

Passport and DL should be good enough. But you need to take a copy your I-495 receipt notice or I-485 approval notice too. If the name was correct on those notices, then it's USCIS's fault and you don't have to pay any fee.
