Card production ordered on March 31, 2008 @ 10:14pm. 
Finally..... I got the email I have been waiting for last 3.5 years. I was stuck in Namecheck since June 28th, 2004. Although the namecheck is still pending but at least I am over with the checking the status daily several times a day.
I want to thank the active members of this forum for their contributions and providing their personal experiences. I learned a lot from other people's experiences and acted upon accordingly.

Vermont to Texas transfer on - 03/02/2007
Last Finger prints - 05/11/2007
4 EADs and 5th one applied on 3/24/2008 - not sure if they cancel and refund the fee, will call.
I-485 approved on 3/31/2008 10:14pm, waiting for physical card.
