Canadians, visas, and the 485 form

The I-485 form part 3A asks for your nonimmigrant visa number, consulate where visa was issued, and date the visa was issued. Even though I am on H1B status, I can't find any visa number and never visited a consulate (was a CoS from TN). I suspect this is because I am Canadian; does this mean I can just leave this section of the form blank, or should I fill it in using my I-94 number and date/place I received H1B status?

You don't need all that other stuff, because you're Canadian. Write down the I-94 number and include a copy of the I-94 and you'll be fine.
