Do I need to report all the canada travel also for the past 5 years in the citizen ship application form ?

Also I traveled to Canada a month ago for 5 days and can I still apply for citizenship ? I read some where I have to be in country minimum 3 months before applying citizenship.

You need to report all trips of 24 hours and longer. But under your best knowledge, means if you can't rember a two day trip 5 years ago it's not a big deal.

You have to reside in the specific state where you apply for at least 3 month.

I did not quite get for my second point. I travelled to canada 30 days ago for 5 days. Do I need to wait my continuous stay of 3 months in US before applying citizenship ?

To make it clear, you need for example live for 3 month in New York State and proof this in case (if they ever ask) with a lease contract, utility bills or something else.

You can't live for example 1 month in New York and 2 in New Jersey when you apply.
 
You don't need to physically be in the US for 3 consecutive months before applying. You only need to be a resident of a state for 3 months, and you can travel abroad during those 3 months. You can even be outside the US for the entire 3 months, as long as you already established residence in the state before leaving, and returned to live in the same state after traveling.

I had close to 20 trips to Canada, all 2-3 days at a time, and I listed all of them. Yes, you are required to list all foreign trips, and Canada is a foreign country
