My wife had RFE for Birth certuficate.
USCIS has asked to submit the Non availability certificate with the school certificate which has parent's name in it.
The problem is: In that certificate the last name of my wife is not there.
Can some one help me with the language of the affidavit stating that
Firstname/ Firstname Lastname is the same person.
Any help in this matter is highly appreciated.
