nothing just fill out ur present name on application and loook for the section where it asks if u want to have ur name changed...
give them the new name...
on interview date,the io is going to confirm the same with u and she is going to update the system....
thts it u r good to go...atleast tht was the process for me

it depends on which d.o you are in...

IIRC namechange requires a federal judicial oath ceremony, so most likely not same day as interview.
