CAN JI be extended with different sponser?

I am orthopaedic surgeon from india.I am getting one year clinical research fellowship in US in John Hopkins medical school.
They are sponsering J1 with option of extending to one more year.
I like to know that if i become board eligible within one year after joining fellowship(i plan 2 finish USMLE) and i get residensy position through ERAS then can my J1 be extended by other sponser or same sponser if at all i get the residensy in same place?
OR i still need to come back to india for two years.
Please enlighten me on this .

If the university is sponsoring your J1, it is in one of the research categories. In order to do a residency, your sponsorship has to come from the ECFMG. The ECFMG sponsors in the 'graduate medical education' category. So you would have to petition the department of state for a change of category. It can be done, but I understand that it can be painful process with uncertain outcome.

If you stay in the same category (e.g. research) you can just get a new sponsor and continue the visa.

I don't want to depress you. But orthopedic surgery is a very competitive specialty in the US. For US medical graduates it is very hard to land a spot. In the last couple of years, only a very small number of foreign graduates have managed to get into ortho residencies over here.
 
If you are getting a research J1 then your best bet is to take all 3 steps of USMLE and go for H1. It kind of depends on the conditions on your J on how they want ot enforce the 2 yr home country residence.
Agreed that ortho is tough, but if you perform well and they like you they might help. Afterall connections are rediculously important in this system. Having said that most of my friends from the surgical specialties are now in FP/IM residencies. Most ortho people go for Physical med/rehab residencies.
