Can I file EAD myself

Hi,
My employer didn't file EAD with my 140/485 filing. Can I file myself? If yes, what is needed. At what stage I can file and what Center?

I have my previous EAD's based on my OPT, under a different A number. The A number that I got on my 485 is different. Also this is my first EAD application after H-1, and 485.
The guide you suggested above shows fields for previos EADs and similar information.

Can I apply online, being the initial EAD and not a renewal?
