Can I apply EAD without I-94 back side

I sent my passport for visa revalidation to US Department of State.

I have the copy of Front side I-94 (original) and not back side and also I have the new H1B extn. approval I-94.

Can I apply for EAD using only the front side of the original I-94?
Does this cause any problem?
