Birth Cirtificate - Is Affidavit needed?

I have a birth cirtificate which I got from the proper authority in India. But the cirtificate does not show the mother's name. Is this cirtificate sufficient for interview at chennai? Or do I have to make an affidavit?. If I have to get an affidavit, what should I have in there for a missing mother's name or that I do not have a birth cirtificate?. Any help is appreciated. Thanks in advance - Ram
 
Any birth certificate must have both the parents names and
mother's name is more important. If this was obtained recently
try and get another one with the proper information.

If that is not possible,you may have to take 2 affidavits, one from your mother and one from your father to support the birth certificate.
