My birth certificate is issued about 10 years after my date of birth. What is the template/ format in which I have to get affadavits made from my parents to prove that it is my real date of birth? Will 2 signed and notarised affadavits from my parents be enough? Thanks.

The birth certificate you have should be enough.

However if you want to play it safe carry one or two affidavits
from your parents.

If you take one it is preferable to take it from mother.

Go to a thread I opened with the name < Sample Form > and
you will find a template with the name 'birth affidavit.jpg '.

Guys,

A question: My birth and my wife's birth were not registered till I visited India last year- this we did specifically for CP. The birth certificate to us was issued by the relevant authorities based on hospital records that was available staing so & so was born on this date. So my birth certificate shows the date of registry of birth as 12/2002 and same for my wife.

Will this be a problem ?? And in any case I also have an affidavit from my parents.

The consulate would have seen several cases like this. This birth certificate and supporting documentation like a school leaving certificate etc. where the birth date is mentioned is sufficient.

In your case you went a step ahead and took the affidavits.
Do the same thing for your wife also, to show in case there are
any questions.
