Birth Certificate

I have a birth certificate stating that the information is taken from original record of birth which is in the register for village/district/state. However the date of registration is not mentioned. It is signed on march 2000. Is is necessary to have the date of registration. Will i get any RFE asking for the birth records.

My mother\'s name is also not mentioned. Is it enough to get a notorised affidavit from my mother. Please give me an idea as to what must be included in the affidavit.
