I am just going to apply for a BC from the Municipality where i was born. If they agree to give it to me , is there any format for the same. This is a case of CP in chennai.

Also if i dont get the BC, shall i get the affidavit on a stamp paper signed by my parents. Would this suffice??

Muncipalities and most other agencies have their own std formats

Just get whatever they give you and it should suffice. If it is not in english, get a translation. If your birth has not been registered get a non availability certificate from the muncipality and an affidavit from parents. Also take your school leaving certificate. If there is no name/different name on the BC also you will need the above.

as previopusly they use to give Birth certificates without name ...only parents name was there ....
 
Question BC from Hospital ????

I have a Birth record from the Hospital which only specifies my
mothers name and not my name. But I do have the 10th Std Certificate
which has my birth date. Will an affidavit from the Indian Embassy in NY do or I need to contat India and get the required documents.
