can any one please post a sample of an affidavit of birth certificate by a relative? I really need as soon as possible>

Piece of Advice. If you are planning to get the affidavit done in India thru one of your relatives, it is better to go my our regular format used in India. If you are planning to get it done here then I think the format which Sertra2002 gave is good. Also if you do it in India better do it in Rs.10 or Rs20 stamp paper. That will have more weightage.
