My wife has a birth certificate however the birth certifcate says that a baby girl was born to so and so on this date. Since the name is not there I would assume I need an affidavit to support this birth certificate. Is that correct?
If so Does the affidavit need to come from the place of birth of from anywhere?
Does it need to be in bond paper/treasury paper?
Any examples of such affidavits available on the web?
