Birth Certificate Name

Hi there,
I have couple of questions.
1. My birth certificate was "registered" only recently. In other words, a birth certificate was issued recently based on an affidavit. So, the birth certificate shows that the birth was registered only in October 2004.
2. My birth certificate has only my mom's first name.
3. My wife's birth certificate does NOT have her name in it. Only her parents name is mentioned.

How do I handle this issue. I was told that the birth should be timely registered and the birth certificate should have parent's first name and last name.
Also, it should have the name of the person on it.

Please let me know what to do. Thanks

If you don't have Non availability certificate, you would need 2 or 3 affidavits from parents and relatives + old proof of your date of birth with both parent names (Ration card, school records etc..) on it.

1) I did the same. No issues . When I got Birth RFE and i submitted this one and got approved.
2) Same thing. ONly mom's first name no issues.


Note that I also submitted affidavits initially when I applied 485. and submitted the above BC when I got RFE

I do not have the copy of "non-availability" and the copy of the affidavit though.
Will that be an issue?

The non-availability and the copy of the affidavit were used to obtain the birth certificate.. Unfortunately, the "ho Jayaga-www.yourmaninindia.com" service that I used did not retain copies of the affidavit and non-availability.

You have to get new affidavits from your parents + 1 affidavit from relative who is old enough to remember date at the time of your birth (What I mean to say is the age gap between you and the person giving affidavit is more than 10 to 15 such as Uncle, Aunt etc.). Also obtain copies of old documentation that contains your name and parent names such as old school records, hospital records etc.
