Birth Certificate Name

Hi there,
I have couple of questions.
1. My birth certificate was "registered" only recently. In other words, a birth certificate was issued recently based on an affidavit. So, the birth certificate shows that the birth was registered only in October 2004.
2. My birth certificate has only my mom's first name.
3. My wife's birth certificate does NOT have her name in it. Only her parents name is mentioned.

How do I handle this issue. I was told that the birth should be timely registered and the birth certificate should have parent's first name and last name.
Also, it should have the name of the person on it.

Please let me know what to do. Thanks

My friend's wife was in the same situation as you mentioned in point 1.
My friend recvd an RFE for his wife's birth certficate, for that he contacted another friend back in India who ran to the municipality office where the clerk wrote on a blank paper something saying that the certifcate is unavailable due to loss of records in a fire etc stamped and signed it.

My friend has his name missing on the birth certificate... I am not sure of this but i think he wrote the name himself, got a photo copy done and sent the photocopy .

I remember him saying that he also got affidavits done by his parents in India as an added evidence.


We did not have any issues with our BCs however, on one of our bcs the year of birth was not mentioned and on another only the first name (instead of full name)of my parents was mentioned,
i managed to get original birth certificates for myself and my wife with the required corrections from the municipality , this was primarily easy because i paid under the table to a clerk who had helped me couple of years ago to speedup the process and my wife's parents knew somebody who worked for the municipal office where her birth was registered,
